Job summary

Catholic Charities Eastern Washington in Spokane, WA is seeking an SMS Operations - Program Support (Full Time) to provide case management and referrals within the SMS Housing Program. The role focuses on assisting residents toward personal independence and housing stability, including assessment, planning, and coordination with community partners.

Responsibilities include client meetings, documentation, coordination with landlords, and ensuring HIPAA compliance.

Qualifications

• BA in social services field or equivalent experience.
• Experience in case management and direct client services.
• Valid Driver’s License and ability to drive for work use.

Responsibilities

• Divert families from homelessness by providing referrals and mediation to resolve barriers to housing stability.
• Conduct Coordinated Entry assessments for families seeking homeless housing services and implement HFCA/Diversion policies.
• Develop individualized Housing Stability Plan (Case Plan) with clients to enable stable housing.
• Create case management files and document services and progress toward goals.
• Coordinate with landlords and act as landlord liaison during tenancy to address issues pro-actively.
• Ensure confidentiality and HIPAA compliance in all PHI handling.
