Job summary

Catholic Community Services of Western Washington is seeking a Case Manager for the Permanent Supportive Housing program in Everett, WA. The role focuses on helping homeless individuals transition to permanent housing and coordinating services with partners.

You will interview residents, assess needs, develop initial service plans, and monitor progress while maintaining accurate HMIS records and collaborating with staff and community providers.

Qualifications

• BA or AA plus direct service experience in case management or related duties.
• Knowledge of substance use disorder and its signs.
• Ability to work with individuals experiencing chronic homelessness.
• Reliable transportation and driving record required.

Responsibilities

• Work directly with clients to secure suitable housing.
• Conduct face-to-face interviews and assess needs to create service plans.
• Monitor client participation in programs and assist with accessing resources.
• Coordinate with community resources, financial assistance, medical care and social services.
• Maintain client records and document progress toward housing goals.
• Provide advocacy and collaborate with housing staff and partners.
