The Housing Solutions Case Manager at Mercy House assists clients in obtaining and sustaining permanent housing through the Housing First model. You will provide case management, housing advocacy, and progress tracking to help clients access Rapid Re-Housing and Homeless Prevention programs.

Under the Housing Solutions Manager, you will maintain client records, participate in team meetings, and collaborate with community partners to expand housing opportunities and services.

Formación

• Bachelor’s degree in social work or related field required
• Experience working with individuals experiencing homelessness is highly preferred
• Proficiency in Microsoft Office and basic computer skills

Responsabilidades

• Provide comprehensive case management services, including intakes, housing stabilization plans, goal setting, referrals, and ongoing monitoring
• Use client-centered, harm reduction, trauma-informed care and motivational interviewing to support housing stability
• Attend meetings with the Housing Solutions team and coordinate services with partners
• Maintain complete and accurate client documentation and case notes in HMIS
• Assist clients with housing searches, connections to landlords, and occupancy support
• Ensure program requirements are met and monitor progress toward housing goals
