Job summary

Mercy House in Santa Ana, CA is seeking a Housing Solutions Case Manager to support residents in the Permanent Supportive Housing program. You will conduct needs assessments, develop individualized housing plans, provide referrals and advocacy, and maintain accurate client records.

Responsibilities include managing a 17–30 client caseload, applying harm reduction and trauma-informed care, coordinating life skills workshops, and partnering with landlords and community resources to sustain housing

Qualifications

• Bachelor’s Degree in Social Work or related field preferred.
• Experience with chemical health, mental health, and co-occurring disorders is preferred.
• Spanish language skills are of significant value.

Responsibilities

• Maintain a caseload of 17–30 individuals and develop housing plans.
• Use harm reduction and trauma-informed care in services.
• Assist residents with credit counseling, job search, and life skills.
• Coordinate workshops and community activities for residents.
