Job summary

Mercy House, Inc. in Santa Ana, CA, is seeking a Housing Solutions Case Manager to join the Permanent Housing Team.

You will conduct ongoing needs assessments, develop individualized housing plans, provide referrals, life skills workshops, and community building activities for residents in the Permanent Supportive Housing Program. The role requires managing a caseload of 17–30 residents, applying harm reduction and trauma-informed care, coordinating with landlords and community services, and

Qualifications

• Bachelor’s degree in social work or related field required or preferred.
• Experience in housing support, case management or social services is desirable.
• Strong written and verbal communication skills.

Responsibilities

• Maintain a minimum caseload of 17-30 residents and develop housing plans.
• Apply harm reduction and trauma-informed care in service delivery.
• Coordinate life skills workshops and community activities.
• Advocate for residents and connect them with community resources.
