Mercy House is seeking a Housing Solutions Case Manager to assist clients in obtaining and sustaining permanent housing through the Housing First model. You will provide comprehensive case management, assessments, referrals, and ongoing monitoring to support housing stability.

Under the Housing Solutions Manager, you will engage in coordination with CES, MDTs, and community partners, maintain HIPAA-compliant records, and help clients overcome barriers to achieve housing goals.

Qualificações

• Bachelor’s degree in social work or related field or 4+ years in social work/human services.
• Experience with homelessness is highly preferred.
• Ability to communicate clearly, both verbally and in writing.

Responsabilidades

• Provide comprehensive case management, including intakes and stabilization plans.
• Maintain accurate notes and client files; monitor progress toward housing goals.
• Advocate for housing options and connect clients to community resources.
• Conduct home visits and coordinate with CES meetings and MDTs.
• Assist with housing searches and landlord relationships.
