Job summary

Daily Planet Health Services in Richmond, VA seeks a Housing Case Manager to provide field and clinic-based case management for individuals impacted by substance use. You will assess needs, develop housing stabilization plans, and coordinate access to housing programs and financial assistance.

You will collaborate with landlords, healthcare and social services providers to promote housing stability and reduce homelessness, while maintaining thorough documentation and client-centered support.

Qualifications

• Experience in recovery work, case management, social services, or housing support roles.
• Knowledge of housing resources and public assistance programs.
• Knowledge of chemical dependency, mental health, and housing/homeless service systems.
• Strong interpersonal and communication abilities to engage diverse clients and stakeholders.
• Ability to manage multiple cases while meeting deadlines.

Responsibilities

• Conduct housing-focused outreach with Daily Planet Health Services and partners.
• Complete housing assessments and individualized stabilization plans with the Housing Navigator.
• Provide ongoing field and clinic case management to support housing retention.
• Coordinate care with prescribing providers, behavioral health clinicians, and other agencies.
• Advocate for clients to resolve eviction risk and secure financial assistance.
• Maintain timely documentation of encounters and outcomes.
