Tri-County Community Action Agency is hiring a Housing Case Manager to assist homeless individuals and families in Southside Virginia. The role focuses on intake, eligibility decisions, and rapid placement into stable housing with connections to community supports.

The position requires strong professionalism, computer literacy, and knowledge of the housing-first approach, plus outreach skills and advocacy for clients in need.

Qualificações

• High level of professionalism.
• Strong computer systems & internet research skills.
• Valid driver’s license/record.
• Knowledge of housing-first approach and stable housing strategies.
• Familiarity with outreach methods, interviewing, assessments, and referrals.
• Broad knowledge of social services and administrative aptitude.
• Ability to advocate for homeless individuals/families with empathy.

Responsabilidades

• Handle intake and screening of homeless clients seeking shelter.
• Determine program eligibility (prevention, diversion, referrals).
• Identify community resources for rapid access to permanent rental housing.
• Provide information, diversion, referrals and shelter as needed.
• Maintain program records (case notes, HMIS, CAP60).
• Apply Voluntary Services model for clients.
