Job summary

LifeMoves is seeking a Case Manager Outreach to support Kaiser patients at Santa Clara hospitals. The role centers on post-discharge planning, housing connections, and coordinating benefits and community resources to stabilize clients.

You will build individualized case plans, advocate for clients, and document progress to improve health outcomes and housing stability through a compassionate, trauma-informed approach.

Qualifications

• Associate degree preferred; or HS Diploma/GED with 2-5 years in related field.
• Experience in housing services, homelessness services, or peer advocacy is valued.
• Trauma-informed and client-centered approach expected.

Responsibilities

• Develop and maintain individualized case plans focused on housing, employment, benefits, and well-being.
• Assist housing searches, employment readiness, benefits enrollment, and budgeting activities.
• Coordinate with healthcare providers and community resources; document outcomes in HMIS.
