Job summary

LifeMoves Guadalupe provides interim supportive housing in San Jose to 96 individuals. The Case Manager reports to the site Program Director and delivers case management, life skills support, and resource connections to help clients achieve housing stability.

Using a trauma-informed, strength-based approach, you will coordinate services, document progress, and partner with clients to set goals, celebrate progress, and overcome challenges.

Qualifications

• Associate Degree preferred; or High School Diploma/GED with 2-5 years of related experience.
• Comfortable using basic technology, including phone, email, Microsoft and Google Suites, HMIS, and databases.

Responsibilities

• Develop and maintain individualized case plans focused on housing, employment, benefits, and well-being.
• Support housing searches, employment readiness, benefits enrollment, and skill-building activities.
• Coordinate with healthcare providers and community resources; arrange transportation as needed.
• Maintain accurate client documentation and outcome tracking in HMIS.
• Advocate for client needs with partners, landlords, employers, or service agencies; provide mediation.
• Provide crisis response and ensure services align with ethical standards.
