Job summary

LifeMoves in San Jose, CA, seeks a dedicated ISH Case Manager to support residents moving toward housing stability. You will develop individualized plans, connect clients with housing, benefits, and health services, and advocate for equitable access with community partners.

Trauma-informed, client-centered approach required. The role requires 2–5 years related experience or an associate degree, strong documentation, and collaboration with staff.

Qualifications

• Associate degree preferred; or high school diploma with 2–5 years related experience in housing services or homelessness services.
• Experience in case management, housing services, or peer advocacy.
• Proficient with HMIS and reporting.
• Comfort with trauma-informed, respectful client interaction.

Responsibilities

• Develop and maintain individualized case plans focused on housing, employment, benefits, and overall well-being, based on each client’s strengths and goals.
• Assist housing searches, employment readiness, benefits enrollment (GA, CalFresh, CAPI) and budgeting.
• Connect clients to healthcare providers and coordinate scheduling/transportation.
• Maintain client documentation and outcome tracking in HMIS.
• Advocate for client needs with partners; provide mediation as needed.
• Build partnerships with community organizations and service providers.
