Job summary

Socket.dev is seeking a dedicated Case Manager to provide direct services to tenants, including assessment, goal planning, and evaluation in both formal and informal settings.

The role involves coordinating with residents, property management and community resources to ensure housing stability, crisis support, and access to health and social services, with at least one evening per week.

Qualifications

• Bachelor's in social work or human services.
• Experience working with vulnerable, at-risk families and mentally ill adults.
• Knowledge and commitment to a Trauma-Informed approach.
• Excellent oral and written communication skills.
• Knowledge of Montgomery County resources preferred.
• This position requires a minimum of one evening per week.

Responsibilities

• Meeting all residents on assigned case load to ensure monthly goal plans and related documents are completed as prescribed.
• Providing referrals and support in maintaining connection to community health services, mental health services, vocational programs, chemical dependence treatment, etc.
• Working with residents and property management to ensure compliance with tenant obligations.
• Assist clients with community activities using personal, public, and agency transportation.
• Crisis intervention and support.
• Leading groups as assigned and participating in on-site events.
• Collaborating to utilize donations and on-site services to meet needs.
• Thorough record-keeping and HMIS data entry.
