Qualifications

  • Education as stated: Bachelor's degree preferred or an Associate degree with relevant experience.
  • Experience in social services, counseling, crisis intervention, or housing services is preferred.
  • Bilingual language ability is highly desirable.
  • Excellent written and verbal communication skills and strong interpersonal abilities.

Responsibilities

  • Provide direct case management including assessment, counseling, crisis intervention, referral and advocacy.
  • Utilize CARES and coordinate with DHS and related agencies to maintain updated client information.
  • Prepare and monitor service plans, progression notes, and incident reports.
  • Advocate for benefits and services to support families toward permanent housing.
  • Coordinate services to stabilize families and address issues such as substance use, parenting, and finances.
  • Document interactions and ensure compliance with state, city, and agency regulations.
