Job summary

Save the Family Foundation of Arizona is seeking a Case Manager to support its Direct Services Program. You will manage a caseload of families, assist with locating and securing rental housing, and help clients complete paperwork and access community referrals toward achieving goals.

Responsibilities include ongoing case management, regular client meetings, service plan development, budget oversight, and HMIS data entry to monitor progress and program effectiveness.

Qualifications

• Bachelor’s degree in social services preferred or five years of related field experience.
• Must be able to speak, read and write English proficiently.
• Demonstrates cultural competence and treats all people with dignity.

Responsibilities

• Ongoing case management to help families secure permanent housing, income, and resources.
• Meet with clients regularly; complete assessments and service plans within program timeframes.
• Develop, communicate and monitor service plan goals with the team and family.
• Ensure budget compliance and manage program expenditures.
• Coordinate client move-ins/move-outs and maintain client communications.
• Maintain organized client files and HMIS data with high accuracy.
