Job summary

Save The Family Foundation of Arizona is seeking a Case Manager for its Direct Services Programs. You will manage a caseload of families, helping them locate rental housing and navigate paperwork to secure stable housing.

In collaboration with clients and partners, you will assess needs, set service plans, monitor progress, and coordinate with housing providers while maintaining HMIS records and program budgets. Evening and occasional weekend availability required.

Qualifications

• High School or GED required; Bachelor's preferred or 5 years related experience.
• Strong cultural competence.
• Proficiency in grammar and spelling.
• Proficient in English reading and writing.
• Valid Arizona driver's license and clean driving record.
• Eligible to work in the United States.
• Flexible schedule including evenings and some Saturdays.

Responsibilities

• Ongoing case management to help families secure permanent housing and resources.
• Meet with clients regularly to complete assessments and service plans within program timelines.
• Develop and monitor service plan goals with the team and families.
• Work with supervisor to manage program budget and approved expenses.
• Act as liaison to community resources and housing providers as needed.
• Ensure timely completion of paperwork and data entry into HMIS and other systems.
• Maintain organized client files and generate program reports on request.
• Coordinate client move-ins/move-outs and maintain team communication.
• Provide customer-service focused engagement with clients, volunteers and staff.
