Job summary

Save The Family Foundation of Arizona is seeking a Case Manager to assist families in locating and securing rental housing, completing paperwork, and coordinating referrals within community programs.

You will conduct assessments, develop service plans, monitor progress, and ensure program compliance while maintaining HMIS data and working within program budgets. This role requires strong communication and cultural competence.

Qualifications

• High School or GED required; a Bachelor’s degree in social services preferred or five years of related field experience.
• Possess strong cultural competence for both cultural and economic characteristics.
• Demonstrate proficiency in grammar and spelling.
• Speak, read and write English fluently.
• Demonstrate cultural competence and inclusivity for all community members.
• Frequent driving to various sites; Arizona driver’s license, insurance and clean record required.
• 21 years of age or older for liability insurance requirements.
• Valid Arizona Fingerprint Clearance Card OR eligible for Level One clearance.
• Eligible to work in the United States.
• Flexible availability; office hours are Mon-Fri 8am-5pm with evenings and some Saturdays.

Responsibilities

• Ongoing case management to help families secure permanent housing, income, and resources.
• Meet with clients, complete assessments, and sign service plans within program time frames.
• Develop, communicate and monitor service plan goals with the team and family.
• Work with supervisor to ensure budget compliance and spending within approved items.
• Assess client needs and barriers; liaise with community to find housing and resources.
• Ensure timely completion of paperwork between tenant, provider, and STF.
• Maintain client communication to monitor progress and meet goals; minimum two in-home visits per month.
• Conduct service team meetings to report progress, updates, and budget status.
• Maintain organized client files; produce reports and HMIS data with high accuracy.
• Collaborate with community to establish referrals and educate on STF programs.
• Record and monitor client data per contract; ensure outcomes and compliance.
• Maintain detailed timekeeping reflecting budget spend-down per grant requirements.
• Coordinate client move-ins/move-outs and ensure team communication.
• Provide customer service to the public, volunteers, clients, and staff as needed.
