Job summary

Far Southeast Family Strengthening Collaborative seeks a Case Manager to coordinate services for families, focusing on earnings and housing stability. You will engage families with Providers, manage housing stabilization plans, and ensure progress while maintaining confidentiality.

This full-time role requires outreach, documentation, and collaboration with supervisors. The Case Manager will conduct home visits, maintain client contact records, and coordinate with education or employment

Qualifications

• BA degree in Social Work, Psychology, Sociology, Counseling, or related service/science or health care related disciplines.

Responsibilities

• Coordinate services for Provider types (including Secondary Service Providers, District Agency Providers, and Community Providers) and ensure families are progressing toward earnings and housing stability.
• Engage families in Provider activities and 2Gen approach; re-engage those who started and paused participation; introduce families to Provider processes.
• Review DHS-administered assessments and develop housing stabilization plans; help families overcome barriers to engagement and participation.
• Develop trusted relationships with assigned families and monitor engagement and progress while in the program.
• Coordinate multiple activities outlined in housing stabilization plans and ensure confidentiality.

Education

BA degree in Social Work, Psychology, Sociology, Counseling, or related service/science or health care related disciplines
