Qualifications

• Minimum 1 year experience working with homelessness and/or mental illness.
• Judicial system involvement or homelessness experience preferred.
• Knowledge of harm reduction and trauma-informed care.
• Valid CA driver's license and good driving record.
• Recent TB and physical examination within past year.

Responsibilities

• Conduct intake assessments for housing, health, and social needs.
• Develop and maintain ISPs, updating as progress occurs.
• Coordinate access to behavioral health, housing, food and transportation services.
• Accompany participants to medical and social service appointments.
• Document case notes, referrals, and care plans in database.
• Track progress and contribute to program reporting.
