Qualifications

• One to two years of experience providing services to highly vulnerable populations.
• Ability to work flexible hours, including early mornings and evenings.
• Access to reliable transportation to meet clients and attend meetings.

Responsibilities

• Provide high quality case management services to clients.
• Evaluate individuals/families for permanent housing.
• Conduct monthly case management sessions to monitor clients’ progress.
• Assist clients with maintaining medication and treatment regimens.
• Develop individualized service plans addressing housing and financial stability.

Skills

Bilingual fluency in Spanish

Excellent organizational skills

Knowledge of housing market and programs

Experience with Harm Reduction

Ability to communicate with diverse groups

Education

Bachelor’s degree in social work, human services, or related field
