Job summary

The People Concern seeks a Temporary Case Manager to support housing stability for Clients as part of the 1603-Housing Retention program at Kensington Campus, Lancaster, CA. You will work onsite with a focus on harm reduction, trauma-informed care, and coordinated services to move Clients from homelessness to permanent housing.

Expect to maintain a ~20-client caseload, collaborate with DMH/DHS, provide life skills coaching, and document outcomes in our secure systems.

Qualifications

• High School Diploma or GED required.
• Bachelor's degree preferred, or 2+ years of experience with homeless individuals.
• Valid CA driver's license with reliable vehicle and insurance.
• Willing and able to transport clients in a personal vehicle.

Responsibilities

• Maintain a caseload of approximately 20 high-acuity Clients for intensive case management (up to 40 for low-acuity).
• Build relationships to support independence and health improvements.
• Coordinate with DMH and DHS programs to align treatment teams.
• Screen, orient, and enroll Clients to program policies and goals.
• Complete biopsychosocial assessments to guide individualized plans.
• Develop quarterly goals to improve quality of life and health outcomes.
• Document progress notes accurately in electronic databases.
• Assist Clients in accessing benefits and coordinating transportation and appointments.
