Job summary

The People Concern seeks an ICMS Case Manager to support Clients’ housing stability through evidence-based practices such as harm reduction, housing first, and trauma-informed care. You will manage a caseload and collaborate with DMH/DHS to coordinate services.

Responsibilities include screening and assessments, goal planning, documentation, and helping Clients access benefits and housing. This is a 100% onsite role in Los Angeles, with a Monday–Friday schedule and full-time employment.

Qualifications

• High School Diploma or GED or equivalent required.
• Valid CA Driver’s license, reliable car, auto insurance, and clean driving record.
• Willing and able to transport Clients in your personal vehicle.
• Able to work in a high tolerance Harm Reduction model with Clients experiencing multiple barriers.

Responsibilities

• Maintain a caseload of approximately 20 high-acuity Clients for intensive case management (up to 40 for low acuity).
• Develop relationships with Clients to support independence and health.
• Coordinate with LA County DMH and DHS programs for integrated treatment.
• Screen, intake, and enrollment of Clients; orientation to policies and goals.
• Complete biopsychosocial assessments to guide treatment planning.
• Develop quarterly collaborative goals to improve quality of life and health outcomes.
• Maintain accurate case notes and electronic records.
• Ensure all Client documents are complete and filed per guidelines.
• Maintain confidentiality per HIPAA and agency guidelines.
• Assist Clients in applying for and obtaining benefits.
• Coordinate appointments, transportation, and follow-up services to improve access to care.
• Direct transportation of Clients in personal vehicle to agencies in the LA area.
• Teach life skills like budgeting, housekeeping, and appointment scheduling.
