Job summary

Adjoin Catalysts seeks a dedicated Specialized Case Manager to support individuals with intellectual and developmental disabilities through housing access, assessments, and individualized stability plans. You'll collaborate with housing authorities, regional centers, and partner agencies to empower clients toward independent living.

You will supervise direct support staff, contribute to ongoing training, and travel within California to meetings and job fairs.

Qualifications

• Bachelor's degree or three years equivalent experience with individuals with disabilities.
• English and Spanish proficiency whenever operationally feasible.
• Knowledge of Title 17 Housing Access Services requirements or related housing resources is preferred.
• Excellent interpersonal skills and ability to follow detailed instructions.
• Certified First Aid and CPR or willingness to obtain within 45 days of hire.
• Insured, registered vehicle and CA driver’s license.

Responsibilities

• Manage a mixed caseload including Housing Access Services participants and other Catalyst programs.
• Process referrals and expedite intake and assessment.
• Provide housing navigation services, including assessment of housing needs and documentation development.
• Coordinate affordable housing resources and educate tenants on eligibility and responsibilities.
• Develop placements and coordinate housing stability plans.
• Supervise, train, and evaluate direct support staff.
• Travel to job fairs, client meetings, and housing sites as required.
