Job summary

Claratel Behavioral Health seeks a Housing Support Case Manager to support adults in permanent subsidized housing, promoting housing stability and wellness through case management and psychosocial rehabilitation. You will coordinate housing search, move-in needs, safety checks, and referrals to community resources.

Work requires in-person and telehealth service delivery, with regular visits, documentation within 24 hours, and collaboration with internal and external providers.

Qualifications

• Bachelor's degree in related field or equivalent education/experience
• One year of experience in a behavioral health setting providing case management/community support service or similar
• Proficient technology skills to use EMR, computer, telehealth system, and cell phone
• Care coordination skills to assist individuals with accessing/linking to additional resources and/or services
• Must have transportation to and from work, strong interpersonal skills and be a positive team player.

Responsibilities

• Carry a caseload (25-30 clients) and conduct a minimum of 1 visit per week per client during the first three months
• Provide care coordination to ensure stability of housing and linkages to services
• Provide ongoing case management and skills training per the treatment plan
• Gather information from client, family, and support system to ensure appropriate level of care
• Recognize the need for crisis intervention and notify supervisor of escalated situations
• Complete timely documentation for all service contacts within 24 hours
• Participate in weekly team meetings and contribute to client treatment needs
• Meet agency productivity standard of 40 billable hours per month
• Meet with program manager for professional growth and evaluation
• Perform additional duties as assigned by team lead/program manager/director
