Job summary

Claratel Behavioral Health is seeking a Housing Support Case Manager to support clients in securing and maintaining housing through hands-on case management and collaboration with service providers. The role includes in-person visits and telehealth, with a Monday–Friday schedule and occasional evenings/weekends as needed.

The position requires a Bachelor’s degree in a related field and at least one year of behavioral health case management experience, plus strong interpersonal and technology

Qualifications

• Bachelor’s degree in related field such as human services, psychology, counseling, sociology or social work.
• 1 year of experience in behavioral health case management/community support services or equivalent.
• Proficient with electronic medical records and telehealth systems.

Responsibilities

• Carry a caseload of 25–30 clients and conduct weekly visits for first 3 months, then twice monthly.
• Provide care coordination to ensure housing stability and linkages to community services.
• Offer ongoing case management and skills training aligned with the treatment plan.
• Gather information from clients and supports to ensure appropriate level of care.
• Recognize crises and notify supervisor/clinical contact as needed.
• Complete timely documentation for all contacts (within 24 hours).
• Participate in weekly team meetings and meet productivity standard (40 billable hours/month).
• Meet with program manager regularly for performance review and growth.
• Perform additional duties as assigned by team lead/manager/director.
