Responsibilities

• Coordinate housing-focused case plans and client care.
• Deliver support services to clients in Shelter, Housing, or Prevention programs.
• Maintain HMIS-compliant case records and reports.
• Conduct home visits and telehealth as needed.
• Collaborate with a multi-disciplinary service team.
