Job summary

Community Housing Innovations, Inc. (CHI) seeks a full-time Case Manager in Hyde Park, NY. You will coordinate services to help clients obtain and sustain permanent housing, develop ILPs, and advocate for resources. The role requires two years in homelessness or housing, and a degree; bilingual skills are a plus.

Responsibilities include client counseling, budgets, referrals, and ongoing documentation with funders. Overtime is available for coverage needs; location is Hyde Park, NY.

Qualifications

• Bachelor’s or Master’s degree preferred; extensive experience may substitute.
• Two years’ experience in homelessness or housing is required.
• Valid driver’s license and valid automobile insurance are required.
• Excellent verbal and written communication in English; Spanish a plus.
• Basic computer skills: MS Word, Excel, Outlook and internet.

Responsibilities

• Ensure participants receive services to promote independence and housing stability.
• Develop with clients an Independent Living Plan (ILP) and monitor progress.
• Provide information, referrals, counseling, crisis intervention, and advocacy.
• Build and maintain a resource network; collaborate with DSS and service providers.
• Assist with budgets, resumes, ADL skills, and direct services as needed.
• Conduct face-to-face meetings; inspect units for safety and compliance.
• Document service use, discharge plans, and reports for funding agencies.
