Job summary

Shelter House in Cincinnati, OH, is seeking a Case Manager to provide intensive case management for individuals experiencing homelessness, focusing on housing, income benefits and prevention of homelessness. The ideal candidate holds a Bachelor’s degree with LSW/MSW preferred, and CDCA preferred. Flexibility to work nights, weekends, or holidays is required. Proficiency with VESTA is a plus.

Qualifications

• Bachelor's degree required; LSW/MSW preferred.
• CDCA preferred.
• Experience with case management, homelessness, mental health or substance abuse.
• Must have a valid driver’s license with daily access to a private insured vehicle and insurer approval.

Responsibilities

• Provide services to clients, including transportation.
• Assist clients with activities that promote achievement of their goals.
• Document client history, updates and goals in VESTA.
• Contact, or attempt contact, with clients weekly.
• Create housing plans for clients (ISP).
• Complete case management process of intake and psychosocial assessment.
• Manage GPD veteran dorms.
• Outreach to veterans; attend VA workgroups bi-weekly.
• Monthly outreach at Hamilton County Justice Center.
• Work with clients matched to the Rapid Rehousing Program (≈50% of time).
