Qualifications

• Experience with vulnerable populations, including trauma and crisis situations.
• Proficient in Microsoft Office Suite (Outlook, Word, Excel).
• Excellent written and verbal communication; ability to work with diverse communities.

Responsibilities

• Screen prospective participants and verify eligibility and documentation.
• Conduct comprehensive assessments and orient participants to program services and rights.
• Develop data-driven care plans and monitor progress to achieve goals.
