Job summary

CP Unlimited is seeking a Case Manager to support residents in achieving housing permanency through empathetic, goal-driven case management. You will build collaborative relationships, provide information, referrals, and coordinate services such as health care, job training, housing, and immigration resources.

Qualifications

• Bachelor’s degree in human services or related field with one year experience in human services.
• Sensitivity to disenfranchised communities including LGBTQ+ and HIV-affected individuals.
• Experience with homeless population preferred.
• Strong written and oral communication skills.
• Ability to work under pressure and respond to crises.
• Flexibility to work evenings and weekends.
• Computer literacy, including Microsoft Office.
• CPR/First Aid and Narcan certification required.
• Bilingual skills preferred (Spanish, French, or Haitian Creole).

Responsibilities

• Case managers carry a caseload of 25 clients.
• Ensure that everyone assigned is personally interviewed, assessed, within 24 hours of admission.
• Conduct intakes and assess residents’ needs and coordinate referrals as needed.
• Develop individualized service plans with residents and review bi-weekly.
• Monitor progress with weekly appointments and bi-weekly ILP reviews.
• Maintain files in an orderly fashion and document missed appointments.
• Work closely with shelter staff to enforce client rules.
• Advocate with agencies to minimize barriers to services.
• Follow up with referral sources to ensure access to services.
• Conduct apartment inspections to expedite re-housing.
