Job summary

Community Economic Defense Project (CEDP) & CED Law is seeking a compassionate Case Manager to deliver comprehensive, client-centered support for individuals experiencing homelessness and housing instability. You will coordinate services, advocate for client rights, and work with a multidisciplinary team to ensure stable housing and access to benefits.

The role emphasizes trauma-informed care, harm reduction, and culturally competent practice, with home visits, documentation in HMIS and HubSpot,

Qualifications

• 1-3 years of experience working with individuals experiencing homelessness, substance misuse, or mental health challenges.
• Knowledge of trauma-informed care, harm reduction principles, and evidence-based practices in substance abuse treatment and mental health.
• Familiarity with community resources, housing programs, and support services for homeless individuals.

Responsibilities

• Conduct comprehensive assessments to identify client strengths and needs, including housing barriers and health concerns.
• Develop individualized care plans with clients, ensuring realistic goals and resource alignment.
• Coordinate services and referrals to substance abuse treatment, mental health services, medical care, and vocational training.
• Advocate for clients and collaborate with external agencies to secure necessary services.
• Provide ongoing emotional support and utilize motivational interviewing techniques to promote positive change.
• Maintain caseload of 15-20 clients with an interdisciplinary team and travel for home visits.
