Job summary

Children’s Services of Roxbury is seeking a Stabilization Case Manager to work with families in shelter as they transition to permanent housing. The role focuses on preventing homelessness and supporting two years of stability for families in Boston.

You will assess family strengths, develop service plans, and collaborate with shelter staff and families to achieve well-being and housing goals. Strong communication and organizational skills are essential.

Qualifications

• Written and verbal fluency in English is required.
• Experience with formerly homeless or low-income families is required.
• Proficiency with data entry and reporting is expected.
• Travel to local meetings is required.

Responsibilities

• Meet with families monthly in their homes across Boston and coordinate with partners to support long-term stability.
• Complete the CSR stabilization assessment and develop an EOHLC service plan to address housing barriers.
• Assist participants in establishing positive relationships with landlords and conduct quarterly check-ins.
• Support families to maintain housing, mediate with landlords, and provide housing counseling and referrals.
• Coordinate legal services and assist with appeals, mediation, and housing court when needed.
• Support families in increasing economic self-sufficiency through collaboration with CSR Workforce Development and budgeting education.
• Assist parents in obtaining health insurance and primary care for all family members.
• Maintain client files and ensure data reporting per HUD requirements.
• Perform other duties as assigned.
