• Advocate and refer clients to benefits and services for transition to permanent housing.
• Coordinate services to stabilize families, address substance use, and improve housekeeping and money management skills.
• Assess client needs, monitor plans, and document interactions for high-quality service delivery.
• Develop relationships with families to support residency at the facility and achievement of service goals.
• Provide individual counseling and crisis intervention to stabilize families during displacement.
• Maintain familiarity with on-site and off-site resources and refer accordingly.
• Facilitate groups to address family issues, life skills, and self-sufficiency for housing transition.
• Coordinate with other staff to ensure timely services and regulatory compliance.
• Ensure effective internal communication and timely transfer of information for admissions and discharges.
