Job summary

Horizon House, Inc. in Philadelphia, PA is seeking a Housing Specialist to work with our Intensive Care Case Management (ICCM) team on housing stability and independence for adults served.

The role focuses on assisting participants to obtain housing, locate resources, and develop self-sufficiency, including budgeting, lease understanding, and transition support, within a trauma-informed, recovery-oriented framework.

Qualifications

• Bachelor’s degree in Human Services or related field with 3 years’ experience.
• Proficient communication and analytical skills; detail oriented.
• Valid driver’s license with acceptable driving record.

Responsibilities

• Assist participants in securing and maintaining housing and resources.
• Conduct assessments to gather information for individualized plans.
• Develop an independent living resource directory.
• Coordinate referrals with housing authorities, landlords, and providers.
• Document services in the electronic health records.
• Participate in team meetings and treatment planning.
