Qualifications

• Bachelor's degree in social work or related field preferred.
• Associate’s degree plus one year of relevant experience acceptable.

Responsibilities

• Provide direct case management, housing location, home visits and follow-up services.
• Guide clients in developing person-centered housing plans addressing income and housing goals.
• Maintain accurate client data in physical and electronic files.
• Enter rent and utility payment data accurately.
• Mediate and advocate with landlords to develop workable housing plans.
• Assist clients in identifying housing and utility assistance programs and referrals.
• Educate clients on residential lease contracts and rights/responsibilities.
• Maintain client confidentiality and professionalism at all times.
• Transport clients within safety protocols for housing and job searches.
• Assist in preparing required program reports.
