Hamilton Health Center in Harrisburg, PA, is seeking a Housing Case Manager to support individuals experiencing opioid use disorder by promoting housing stability, recovery, and long-term well-being.

In this role you will provide case management and care coordination services, helping clients navigate housing resources, connect to community supports, and achieve individualized recovery goals, with a focus on advocacy and compassionate service.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or a related field or progress toward within 6 months
• Minimum of 1 year of case management experience preferred
• Experience with substance use disorder, housing services, or vulnerable populations is a plus

Responsibilities

• Provide case management, care coordination, and advocacy to support client recovery and housing stability
• Develop and implement individualized service and housing plans with clients
• Connect clients to housing resources, employment support, counseling, and community services
• Maintain relationships with landlords and community partners to secure housing opportunities
• Monitor client progress and adjust service plans as needed
• Document all services and interactions accurately and in a timely manner
• Support clients in overcoming barriers to care, including transportation needs
• Collaborate with healthcare providers, agencies, and community organizations
• Participate in required meetings, trainings, and outreach activities
• Ensure compliance with confidentiality, program guidelines, and regulatory requirements
