Access Services is seeking a compassionate Housing Case Manager in Fort Washington, PA, to empower individuals toward stability. This full-time on-site role blends mental health and housing stabilization, coordinating between clients, landlords, and community resources.

Responsibilities include developing IRPs, updating Housing Stability Plans quarterly, and maintaining thorough documentation. Candidates need a relevant degree or Certified Peer Specialist, with PRP certification due within 12

Qualifications

• Bachelor’s degree in human services, social work, psychology or special education required.
• Certified Peer Specialist certification required.
• PRP certification must be completed within 12 months of hire.
• Some related mental health services internship/experience preferred.
• Experience in supportive housing systems preferred.
• Strong communication and interpersonal skills essential.
• Knowledge of mental health, homelessness and housing systems.

Responsibilities

• Meet referred individuals with the Housing Locator to assist move-in.
• Develop and update Housing Stability Plans quarterly with clients.
• Maintain documentation for move-in, recertification, and ongoing cases.
• Conduct monthly home visits and support during transitions.
• Coordinate transportation for health appointments and job interviews.
• Identify barriers to housing stability and devise solutions.
• Connect clients to social, health, and benefit resources.
• Ensure HUD compliance and funding guidelines.
• Participate in county meetings and case reviews.
• Develop and update IRPs for rehabilitation planning.
