Qualifications

• 3 years of direct care experience or equivalent combination of education and experience.
• Valid driver's license with an acceptable driving record is required.
• Availability for local travel necessary.

Responsibilities

• Ensure delivery and flow of appropriate services for clients.
• Participate in development of individual rehabilitation plans.
• Assess service delivery gaps and recommend alternative plans.
