Job summary

Mercy House is seeking a Housing Solutions Case Manager to provide needs assessment, emergency housing plans, referrals, and advocacy for residents in the Permanent Supportive Housing Program. This role emphasizes trauma-informed, client-centered practices with accurate record-keeping.

Responsibilities include managing a caseload of 17–25 individuals, coordinating with landlords and community resources, and on-call availability.

Qualifications

• BA/BS in Social Work or related field (or equivalent experience)
• 2+ years of lived experience with homelessness/behavioral health welcomed
• Valid CA driver’s license, proof of insurance
• Ability to plan, organize, and prioritize duties

Responsibilities

• Maintain a caseload of 17–25 individuals and meet residents at least twice monthly to develop emergency plans and housing support plans.
• Utilize harm-reduction and client-centered approaches to help residents stay housed.
• Conduct health and safety visits, inspect housing, and coordinate with Leasing/management teams.
• Coordinate with landlords and community resources to link residents with employment, education, tutoring or mentoring.
• Document all case notes, plans, and interactions in a timely, accurate manner; participate in on-call schedule as required.
