Job summary

Mercy House in Santa Ana, CA seeks a Housing Solutions Case Manager to assist clients in obtaining and sustaining permanent housing using the Housing First model. You will conduct intakes, develop housing stabilization plans, and monitor progress toward housing goals with empathy and evidence-based methods.

The role emphasizes trauma-informed care, motivational interviewing, and collaboration with CES meetings, landlords, and service providers.

Qualifications

• Bachelor’s degree in social work or related field and/or four years of experience.
• Experience working with homeless populations is highly preferred.
• Ability to communicate clearly, organize duties, and intervene in crises when needed.

Responsibilities

• Provide case management services including intakes, housing stabilization plans, and goal setting.
• Advocate for clients and coordinate housing resources and referrals.
• Maintain case notes, program logs, and HMIS data in a timely manner.
• Collaborate with landlords, service providers, and tenants to sustain housing.
