Job summary

Step Up in Santa Monica, CA is seeking a Service Coordinator I to support intensive housing services for tenants in PSH and community housing. You will engage in case management, coordinate lease-ups, complete intake, and help tenants achieve independent living goals on a 40-hour week, non-exempt basis.

Responsibilities include on-site supportive services, data entry in CHAMP/HMIS, collaboration with Tenant Manager, and adherence to Housing First principles while maintaining tenant safety and

Qualifications

• Minimum of 2 years’ experience working with people who experience homelessness and/or are diagnosed with a mental illness.
• Bachelor's degree in Social Work or Mental Health related field; 1+ year experience in community housing settings.

Responsibilities

• Participates in referral processes to screen for housing eligibility.
• Coordinates lease up activities with Tenant Manager and provides tenant orientation.
• Prepares applications and coordinates with referrals for lease up and interviews with property management.
• Performs tenant intake during first week of move-in.
• Develops initial and quarterly service plans for each tenant.
• Meets tenants weekly and monthly; increases frequency as needed.
• Maintains documentation and charting; manages files and daily logs.
