Job summary

Step Up on Second, Inc. is seeking a Service Coordinator I - Generalist to support outreach, housing linkage, and ongoing case management for individuals experiencing homelessness in Sacramento.

The role evolves into SCI-Care Coordinator for housing retention as participants stabilize, with an emphasis on compassionate engagement and data-driven service delivery. The position requires strong collaboration with Homeless Services Program staff, daily outreach, and documentation in HMIS/EHR

Qualifications

• BA in psychology or social work or 3 years related work experience.
• Experience with homeless population essential.
• Document progress and maintain records per program standards.

Responsibilities

• Conduct street outreach and engagement for motel voucher and housing program.
• Assess mental health and co-occurring needs and link to services.
• Complete VI-SPDAT surveys and submit results for housing match.
• Provide crisis intervention and triage, connect to housing/health resources.
• Document services within required timeframes in HMIS/EHR systems.
• Maintain participant files and assist with data/reporting.
• Coordinate with City Departments to support education and linkage to services.
