Job summary

Step Up is seeking an Intake Services Coordinator I for Community Supports-ILOS to engage referred members and coordinate housing assistance as part of the Cal-Aim program. The role involves field-based outreach, housing navigation, and collaboration with a care team to improve housing stability and health outcomes.

The position requires an AA degree or equivalent and a valid California driver's license. Bilingual English-Spanish is preferred; you will work under the Program Manager and Director

Qualifications

• AA degree or equivalent.
• A valid California driver’s license and clean driving record.

Responsibilities

• Outreach and engagement to enroll referred members in ILOS-CS.
• Housing navigation and transition services.
• Housing deposits/backup funding coordination.
• Housing tenancy and sustaining services coordination of care.
• Advocacy on behalf of ILOS-CS members in home, community, and provider settings.
• Coordinate with landlords and community partners to support successful tenancy.
