Job summary

Step Up on Second, Inc. is seeking a Service Coordinator I for the Community Supports-ILOS Program.

Under the supervision of the Program Manager, you will provide high quality care management, outreach, housing navigation, and coordination of services to improve members’ housing stability and health outcomes. This hourly position requires an AA degree (or equivalent), 2+ years of case management, a valid California driver's license and reliable vehicle; bilingual English/Spanish preferred.

Qualifications

• AA degree or equivalent.
• 2+ years of case management experience or related work preferred.
• California driver’s license and access to reliable vehicle are required.

Responsibilities

• Provide high-quality, field-based care management and care coordination.
• Outreach, engagement, and housing navigation to promote tenancy stability.
• Coordinate with health plans, providers, and community resources to improve outcomes.
