Job summary

Mental Health America of Los Angeles (MHALA) is seeking a Personal Service Coordinator/Intensive Case Manager (Temporary) to provide high-quality ICMS to clients eligible for permanent housing through the HFH program. You will deliver client-centered services, sensitive to homelessness-related challenges as they move toward stability.

Responsibilities include intake, enrollment, psychosocial assessments, linkage to health and housing resources, and collaboration with DHS and HFH partners.

Qualifications

• High school diploma or GED with 2 years’ experience in psychosocial rehab-related services.
• Bilingual Spanish speaking preferred.
• Must function well in a team-oriented approach to client services.
• Proficient in Microsoft Word, Excel and Outlook.
• Be able to learn HMIS and Electronic Health Records systems.
• Experience with mental illness, chronic health issues, and substance use disorders.
• Flexible schedule including some nights, weekends and holidays.
• Valid driver’s license, vehicle, and insurance requirements.

Responsibilities

• Provide case management support to a caseload of clients transitioning to permanent housing.
• Assist with housing stabilization and move-in processes and associated documentation.
• Coordinate referrals from DHS, CES partners, and HFH partners; establish rapport with clients.
• Conduct intake, enrollment, and eligibility documentation; assist with housing applications.
• Link clients to health, mental health, and substance use services; maintain medical home continuity.
• Assist clients with income, benefits, employment, budgeting, and transportation needs.
