Job summary

Step Up is seeking a Service Coordinator I for its Santa Monica PSH programs. The role focuses on intensive case management, housing retention, and coordination with tenants, property management staff, and program leadership.

You will conduct intake, develop service plans, and provide on-site supportive services, including crisis intervention and community integration, to tenants in PSH and community housing. Strong collaboration and documentation skills are essential.

Qualifications

• Minimum of 2 years’ experience working with people who experience homelessness and/or are diagnosed with mental illness.
• Bachelor's degree in Social Work or Mental Health-related field and at least 1 year experience in a community or housed setting.
• Excellent written and verbal communication skills; ability to perform extensive charting and documentation.

Responsibilities

• Engage tenants in Intensive Case Management Services (ICMS) to improve independent living skills and housing retention.
• Coordinate lease-up activities and provide orientation on housing access and services.
• Develop and monitor individualized service plans; meet tenants weekly and document progress.
• Maintain tenant records and ensure timely data entry in HMIS/Clarity.
• Provide on-site supportive services, crisis intervention, and community integration activities.
