Job summary

Qualifications

• High school diploma or equivalent required; Associate’s degree preferred.
• Bi-lingual/Spanish proficiency preferred; experience with underserved populations desired.
• Experience with referral coordination, case management, or related social services/housing knowledge is beneficial.

Responsibilities

• Act as central coordinator for all referrals to agency programs.
• Review, process, and prioritize referrals based on eligibility and client needs.
• Enter and update referral data into client management system with accuracy.
• Collaborate with case managers and program staff to assess eligibility and placements.
• Provide information about programs and assist clients through intake and referrals.
• Track referral status and assist clients through intake for smooth service transitions.
• Maintain communication with external providers to ensure coordinated care.
• Develop relationships with community partners and housing authorities.
• Provide timely updates and follow-up on referrals to ensure appropriate services.
• Maintain detailed records of referrals, outcomes, and follow-ups.
• Ensure compliance with privacy regulations (HIPAA) and documentation standards.
• Stay current on available housing programs and resources in the community.
• Assist staff with documentation for new client intakes.
• Identify trends and areas for process improvements; report to leadership.
