Job summary

Breakthrough is seeking a Case Manager Supervisor, Housing in Chicago, IL. The role oversees Transitional Housing, Family Housing, and Permanent Support Housing Case Managers, coordinating with multiple departments and guiding staff in case management, tenant support and resource acquisition for Breakthrough’s programs.

Qualifications

• Bachelor’s degree preferred in a Human services field.
• Minimum of two years supervisory or equivalent leadership experience.
• Strong knowledge of program outcomes and deliverables.
• Ability to relate well to and coordinate with guests and program participants; promote customer care.
• Develop and articulate a strategic vision for the program area.
• Project a professional image to internal/external customers with proper grooming.
• Demonstrate initiative and follow through; comfortable in a changing environment.
• Willingness to improve work performance.

Responsibilities

• Supervise, manage, monitor, train and coordinate activities of all assigned staff using approved performance management processes and policies.
• Train team on all service delivery models and ensure quality delivery.
• Provide detailed statistical and narrative reports to the Program Director.
• Attend meetings with external program partners and internal departments.
• Complete reports for funding agencies on a routine basis.
• Enter all staff data into the database daily.
• Establish and monitor internal quality management systems.
• Follow agency policies and procedures independently.
• Complete other related tasks as assigned and required.
