Job summary

Breaking Ground, a nonprofit dedicated to ending homelessness, is seeking a Case Manager, Outreach to support individuals experiencing homelessness in transitioning to permanent housing. Reporting to the Team Leader, you will manage a caseload of about 25 clients, perform assessments, assist with benefits, and coordinate services across housing, medical and mental health supports.

This 35-hour week position emphasizes harm reduction, motivational interviewing, and strong written and verbal

Qualifications

• Bachelor’s degree or equivalent work experience.
• Two years of experience with homeless adults preferred.
• Harm reduction and motivational interviewing experience preferred.
• Valid driver’s license in NY/NJ/CT/PA with clean record.
• Proficiency in Microsoft Word and Excel; ability to learn new programs.
• Excellent writing and communication skills.
• Bilingual Spanish/English preferred.

Responsibilities

• Provide direct services to a caseload of approximately 25 clients, including assessments, benefits assistance, and services to support housing and recovery goals.
• Conduct psychosocial evaluations.
• Connect clients to psychiatric and medical services as appropriate; coordinate treatment.
• Assist clients in obtaining identification, benefits, and income sources.
• Meet weekly with clients to assess needs and develop service plans.
• Prepare housing applications and assist with permanent housing processes.
• Maintain records in compliance with agency and state standards.
• Intervene in crisis situations and escort clients to appointments as needed.
• Train to assist with self-administration of medications per policy.
