Job summary

Homeward Bound Admin in Novato, CA is looking for a Program Manager - Sweeney Place to provide housing-focused case management and support residents toward stability and independence.

You will coordinate housing supports, develop individualized plans integrating housing, medical, behavioral health, and services, and maintain records in HMIS and Salesforce while ensuring CalAIM/HIPAA compliance. The role includes supervision and collaboration in a fast-paced environment.

Qualifications

• Bachelor's degree in social work, psychology, human services or related field, or 3 years in housing/human services.
• Experience with homelessness or underserved populations preferred.
• Knowledge of Housing First, trauma-informed care, and community resources.
• Experience with HMIS or Salesforce and Microsoft Office.

Responsibilities

• Provide housing-focused case management to individuals and groups.
• Coordinate housing support services, CalAIM and referrals.
• Develop and monitor individualized housing and care plans.
• Oversee housing referral, interview, placement, move-in, and retention.
• Maintain records in HMIS/Salesforce and ensure compliance.
• Monitor progress, address barriers, provide crisis intervention, and coordinate resources.
• Support property management activities and supervise staff.
