Job summary

Homeward Bound of Marin seeks a dedicated housing-focused case manager to support individuals and families experiencing homelessness in Marin County. You will coordinate housing support services, develop individualized housing and care plans, and oversee referral, placement, and retention processes.

Ideal candidates have a Bachelor’s degree in social work or related field (or 3+ years in housing or human services), experience with Housing First, CalAIM, and HMIS/Salesforce, and strong

  • Bachelor’s degree in social work, psychology, human services, or related field, or 3+ years of relevant experience.
  • Experience working with individuals experiencing homelessness or other underserved populations preferred.
  • Knowledge of Housing First, harm reduction, trauma-informed care, mental health, substance use, and community resources.
  • Familiarity with public benefits systems (SSI, SSDI, SDI) and housing programs.
  • Experience with HMIS, Salesforce, or similar databases, and proficiency with Microsoft Office and cloud-based applications.
  • Strong communication, organization, documentation, and case management skills.
  • Ability to work independently and collaboratively in a diverse, fast-paced environment.
  • Commitment to equity, inclusion, confidentiality, and professional standards.

Responsibilities

  • Provide housing-focused case management to individuals and groups.
  • Coordinate housing support services, CalAIM (ECM and Community Supports), referrals, and service eligibility.
  • Develop and monitor individualized housing and care plans that integrate housing, medical, behavioral health, and supportive services.
  • Oversee housing referral, interview, placement, move-in, and housing retention processes.
  • Complete accurate and timely documentation, maintain client records in HMIS and Salesforce, and ensure compliance with CalAIM, HIPAA, agency, and funder requirements.
  • Monitor participant progress, address barriers to housing stability, provide crisis intervention, and coordinate appropriate community resources and referrals.
  • Support basic property management activities, including maintenance coordination, tenancy issues, voucher processing, and site monitoring.
  • Supervise and support assigned staff, including training, coaching, and performance oversight.
  • Participate in supervision, meetings, trainings, quality improvement activities, and perform other duties as assigned.

Homeward Bound is Marin County’s primary provider of shelter and housing for individuals and families experiencing homelessness. We offer emergency shelter, short- and long-term supportive housing, job training and placement, and wraparound services. Our mission is Opening Doors to Safety, Dignity, Hope, and Independence.

What You’ll Do
  • Provide housing-focused case management to individuals and groups.
